Enrollment Application - 
 
An enrollment application is required for all students. Enrollment is encouraged to be completed using our online enrollment portal. Paper applications are available upon request. We provide translated enrollment packets in Arabic, Simplified Chinese, Korean, and Spanish. If you would like an enrollment application in another language not offered, please contact your child's school. You may also email [email protected] or call the district office at (509) 332-3581.
 

 
Documentation of Student Age
 
Documentation of student age is required for all new students.
  • Kindergarten students must be 5 on or before August 31.
  • First grade students must be 6 on or before August 31.
Acceptable documents include a birth certificate, foreign birth certificate, passport, adoption record, previously verified school records, a religious, hospital, or physician's certificate showing date of birth, an entry in a family bible, or any other documents permitted by law.
 

 
Student's Medically Verified Immunization Records
 
All Washington State public schools are required to meet the student immunization guidelines from the Washington State Department of Health. Students will be ineligible to attend school if appropriate immunization records are not provided to their school. 
 
We encourage families to plan ahead! Please consider reaching out to your student's medical provider to request a copy of your student's immunization records. A copy of your student's medically verified immunization record can be uploaded directly into your online application. Families may also opt to email, main, fax, or drop off a physical copy of the immunization record at the student's assigned school (or at the district office during summer hours when the schools are closed). 
 

 
Ages & Stages Questionnaire (ASQ) - Required for Kindergarten Only
 
The Ages and Stages Questionnaire is a screening tool that is used for all kindergarten students. This questionnaire takes a snapshot of your child's current skills and may help identify if your child is at risk of a delay. It provides a fast and helpful look at how your child is doing in important areas like communication, social skills, motor skills, and problem-solving skills. This will help our educators prepare for how to best support your child and to plan the next steps when it makes the most difference - your child's critical first 5 years of life.

Note for Families Experiencing Homelessness -
