Online Requirements

• Athletic Registration – now through FinalForms! 
• Emergency Contact Form
• Inherent Risk for each sport. Please complete all the inherent risk forms for the sports your child wants to participate in for the ENTIRE year.
• ASB Card Purchase

Paper Requirements

• Physicals (needs to be updated every two years)

Impact/Concussion Testing

• One-time use link sent to your student’s school email account
  • If they have not completed it, coaches will set them up during practice to do it
